I have never seen the lifter end fail like that, "hollow point" pretty much sums it up. I have had several failures on the cup end though. My experience has been that 3/8" pushrods with cups the same diamter as the pushrod are more durable than 5/16" pieces with the wine glass type cup. Just a thought.
