Find a '74-'76 Mopar A-body, (Dart, Duster, Demon, Valiant), in a salvage yard that has front disc brakes. Get everything from where the upper control arm fastens to the inner fender panel, down to the lower ball joint. Yes, you MUST get the upper control arms. Also get the master cylinder and the proportioning valve. This is pretty much a straight-across, bolt-on swap, but you are advised to trade in the calipers and master cylinder for fresh rebuilt units, and clean up the rotors.

I did this swap earlier this year on my '64, and am very happy with it. I combined it with a front end rebuild/upgrade.
There are several sites online that give details of various approaches. None are perfect, and they don't agree with each other. Here's one:
http://www.moparaction.com/Tech/archive/disc-main.html
A couple of books that might help:
http://www.earlycuda.org/tech/books.htm#2
My setup uses the following:
Original lower control arms (with homemade reinforcement plates), strut rods, etc.
'74 Duster upper control arms, knuckles, dust shields, rotors, balljoints.
"340" torsion bars from an unknown A-body.
Moog HD rubber bushings.
'80 Volare calipers and hoses.
'76 Valiant Disc/drum master cylinder.
Adjustable brake proportioning valve.
C-body tie rods.
15X7 cheapie rims for now, with a set of rallyes in my garage waiting for some attention.
The parts came from junkyards, ebay, www.rockauto.com, Summit, NAPA, and my magic hardware cans.
